After an extensive national search, Charkes Nesbitt was selected to be the strategic leader for all Case Management, inpatient & outpatient, programs supporting the transition from a fee-for-service, volume-based care delivery system to a value-based care model with a particular focus on JPS Connect patients and across multiple payors such as Medicaid, Medicare, and Medicare Advantage.

Charkes comes to JPS from WellMed Medical Management, an Optum company in Houston, Texas. Prior to WellMed, Charkes was with Jackson Health System in Miami, Florida, a 6-hospital, a clinically integrated health system with over 2,100 beds for 6 years in multiple progressive leadership roles.
Ms. Nesbitt is an advanced practice nurse and case management leader dedicated to developing and operating comprehensive case management programs that improve utilization of resources, care coordination, as well as transitions and quality of care. Charkes has a Bachelors in Science, Nursing from Bethune-Cookman and a Master of Science in Registered Nursing (Acute Care Nurse Practitioner) from the University of Florida with supporting certification in Case Management from the Commission of Case Managers.
